Platform for Improving and Optimizing the Management of Chronic Patients, Based on Artificial Intelligence Techniques and Non-Invasive Monitoring
Summary
The objective of SMARTCHRONIC is to develop tools that optimize the definition and deployment of integrated care pathways for chronic patients, depending on their frailty and long-term care needs. These pathways require multidisciplinary interventions from a proactive, patient-centered perspective.
Description
The innovative nature of the project is based on three key aspects:
- Development of a tool that uses Artificial Intelligence to model and predict the evolutionary behavior of the frailty index, characterizing care pathways and resources based on this index and its evolution.
- Creation of a predictive model that analyzes the risk of hospital readmission in chronic patients, using historical patient records, including the frailty index and its relationship with care pathways.
- Incorporation of an innovative non-invasive monitoring system through mobile technology (mHealth), which enables the measurement and modeling of the digital phenotype.
The project's results will strengthen the current healthcare system for patients requiring comprehensive care, which has been overwhelmed in recent years due to the increase in patients with advanced-stage chronic diseases.
SmartChronic, with a 23-month execution period and a total budget of €684,000, is supported by funding from the Agència Valenciana de la Innovació (AVI).
